• Utah Immunization Requirements

• Media Opt-Out Form (if desired)

• Bring the completed documents to the main office, along with:

  • Original birth certificate of the student

  • Original immunization record

  • Guardian photo I.D.

  • Two

    Proofs of Residency (See chart on Proof Of Residency Form)
